Medical Equipment Form

To ensure that Sumter EMC is aware of your medical equipment needs, please complete and submit the Medical Equipment Form.

This form helps the cooperative keep your account flagged for special consideration during outages or planned maintenance.

No utility can guarantee uninterruptible service, so if you or a family member depends on a medical life-support device, you are responsible for providing a back-up source of power for the medical device or having a plan to evacuate to a facility that is not affected by the power outage. 

Please ensure we have a copy of the form below if you require medical life-support equipment.
